1. What types of benefit plans are available to my clients?

We offer HMO type of benefit plans and Point of Service benefit plans.

2. How many different plans do you offer?

Six (6) HMO type plans and six (6) Point of Service Plans

3. Do you have any out-of-network benefits?

Yes. All Point of Service Plans offer out-of-network benefits, and are also available to employees who live or work in LaGrange, Georgia.

4. Do you provide prescription drug benefits?

Yes. We have several different plans to accommodate your needs. The prescription drug benefit is purchased separately as a rider.

5. Who is in your network?

We have over 300 physician providers and hospitals located in Columbus, Georgia, LaGrange Georgia, Atlanta Georgia, Phenix City Alabama, and Opelika, Alabama.

6. What is the smallest / largest group your can cover?

We can offer group healthcare benefits to a group as small as two (2) employees. There is no limit on the largest group we can cover.

7. Do you sell individual policies?

No, we do not sell individual policies, but we would be happy to refer you to an agent that we work with, that can sell you an individual policy.

8. Where can I get a copy of the benefit plans?

9. How do I submit a census?

Fax your census to (706) 660-6515 or email it to us.

10. How much commission do I earn on my groups?

There is a sliding scale for commissions. Groups 2-29 employees pays 7%. Please call our office for more details about commission.

11. How do I get appointed to sell EHP products?

You must complete a “Producer Agreement” and be appointed by the Georgia Department of Insurance. This is usually about a 45 day process.

12. What is the last day of the month I can submit a group to be covered for a first of the month effective date?

EHP will accept a Group Application up until the close of business on the fifth (5th) day of the month for a first of the month effective date. Any group information received after that deadline will be effective the first day of the next month.

13. Does EHP do COBRA administration for employers?

Yes, we can administer COBRA for any employer group that has twenty (20) or more employees insured with Evergreen.

14. Do you have coverage available out-of-state?

No, we do not. Our license with the Department of Insurance states that employees must be able to access medical services in our service area.

15. Where is Evergreen Health Plan, Inc. located?

We are located at 707 Center Street, Suite 110, Columbus GA, 31902, (706) 660-6175.

16. Who owns Evergreen Health Plan, Inc?

A partnership with Columbus Regional Healthcare Systems and a group of 100 physician investors make up the ownership of EHP. EHP is the only locally owned, operated and managed company offering healthcare benefits in Columbus and the surrounding areas.
